Once your membership is activated, you will be an official member of the 2013 Jr. Jays Club. Activation benefits include a subscription to ACE™'s E-Newsletter and access to exclusive Members-Only events such as a batting practice visit, an autograph sessions and more.

IMPORTANT – Activation form MUST be completed by the Parent/Guardian of the Jr. Jays Club Member.

The Jr. Jays™ Club (the "Program") is open to children aged 14 and under only. PARENT/GUARDIAN CONSENT IS REQUIRED FOR A CHILD TO JOIN THE PROGRAM. By entering the requested credit card information on this form, the parent/guardian consents to all rules, policies and conditions applicable to the Program, as set by Rogers Blue Jays Baseball Partnership (the "Club"), owner of the Toronto Blue Jays™ Major League Baseball team (the "Team"), from time to time.

The Club agrees that it will use any information provided by the parent/guardian in respect of the child (the "Child's Information") only to administer the Program, to communicate with the child in respect of the Team, the Program and/or family-oriented initiatives from time to time (e.g. Jr. Jays™ Saturdays) and to improve the services it provides to the child (the "Purposes"). The Team will not rent or sell this information to any third party. The Club will only disclose the Child's Information to a third party where required by law to do so or where the disclosure is reasonably necessary for the purpose of assisting the Club with the Purposes and the information disclosed is protected by an obligation of confidentiality.

Please note that the bluejays.com website is operated by MLB Advanced Media, LP ("MLBAM") and that MLBAM will be (i) hosting online sign-up forms for the Program, (ii) receiving information submitted online in respect of the Program, and (iii) sending electronic communications related to the Program and the Purposes on behalf of the Club. MLBAM will only use the Child's Information for the Purposes and not disclose such information to any third party unless required by law to do so or where the disclosure is reasonably necessary for the purpose of assisting the Club with the Purposes and the information disclosed is protected by an obligation of confidentiality.

Unless the parent/guardian provides his/her consent to receive marketing communications from the Club and/or MLBAM, the Club and MLBAM will only use the parent/guardian information for the Purposes (for example, the Club may send information about the Program to the parent/guardian in place of the child and may use third parties to assist the Club with the Purposes).
